We at SHAPE are committed to offering Atlanta the most affordable, convenient, and healthy weight loss solutions available. We believe so strongly in our products and services that we created the SHAPE WELLNESS REWARDS PROGRAM. Over the past year we have received an incredible amount of support from clients who generously have spread the good news about SHAPE and our success stories. In appreciation of those individuals and in celebration our outstanding performance in the Greater Atlanta Area - SHAPE Medical Wellness Center is pleased to offer the following referral based incentive to all of our current and future clients:

Refer 1 Patient to Shape Wellness - $75.00

Refer 2 Patients to Shape Wellness- $150.00

Refer 3 Patients to Shape Wellness- $225.00

Refer 4 Patients to Shape Wellness- $300.00

Refer 5 Patients to Shape Wellness- $375.00

Refer More than 5 patients and you will receive $100.00 per patient with no cap/limit on the number of patients.

To qualify for the cash payment a participant/referral must adhere to the following criteria:

1. You must be a US citizen and be willing to complete the appropriate federal and state tax documentation. 2. All clients referred must purchase at least 150.00 of non-refundable goods and/or services on their first visit. 3. Cash payments will be made on the 1st of each Month. Cash payments will only be made on clients that have no outstanding balances to SHAPE. 4. Members of the Rewards Program will receive a ONE TIME pay-out for each referral; there will be no payments made on referral return visits. Additionally, under no circumstances will multiple payouts be made for one individual. 5. Any disputes as to who made a referral will be decided solely by shape management. 6. You must be a CURRENT client of Shape Medical Wellness Center and currently enrolled in one of our programs. 7. Referral Payment period will be from the 1st to the 30th or 31st of each month. Checks will be written and mailed to each Rewards member no later than the2nd of each month. There will be NO cash payments or advances given, all referral payments will be mailed through the U.S. Postal Service.

You must register with our offices as a participant prior to receiving any cash payments for referrals.
